USERRA Health Care Coverage Extension Act of 2004 - Extends from 18 to 24 months the maximum period of employer-sponsored health care coverage that a person covered by the Uniformed Services Employment and Reemployment Rights Act of 1994 (USERRA) may elect to receive while absent from employment due to service in the uniformed services.

Reinstates reporting requirements for the Secretary of Labor relative to USERRA cases.
