Community Hospital Preservation Act - Authorizes the Secretary of Health and Human Services to make grants to eligible nonprofit community hospitals for legitimate hospital purposes.

Sets forth provisions for: (1) the recovery of grant amounts if they are not used for legitimate purposes, within ten years, or by a hospital meeting eligibility requirements; and (2) forgiveness of repayment of ten percent of a grant amount for each year the recipient met one or more eligibility requirement.

Mandates an annual report by the Secretary to Congress on the activities carried out under this Act. Authorizes appropriations.
