Rural Medical Emergencies Air Transport Act of 1991 - Amends the Public Health Service Act to direct the Secretary of Health and Human Services to make grants to at least seven States to assist in the creation or enhancement of air medical transport systems that provide victims of medical emergencies in rural areas with access to treatments.

Requires: (1) preference for States with plans to integrate the proposed air medical system into a comprehensive network or plan for regional or statewide emergency medical services; and (2) the grant to be administered by the State agency responsible for medical emergency or trauma services programs. Allows the State to award grant funds to public and private entities.

Authorizes appropriations.
