Child Health Program Act of 1991 - Amends title XIX (Medicaid) of the Social Security Act to require State plans for medical assistance to make available through a mail application process preventive care services and outpatient and physicians' services for children whose parents do not have health insurance but have incomes between 133 and 185 percent of the Federal poverty line and choose, via an annual enrollment fee, to participate in such child health program. Finances such program entirely through Federal funding.

Amends the Internal Revenue Code to finance such program by removing the cap on the maximum amount of income subject to the Medicare hospital insurance (part A of title XVIII of the Social Security Act) payroll tax.
