Family Caregiver Support Act of 1991 - Amends the Social Security Act to add a new title XXI entitled "Grants to States for Family Caregiver Support Programs" to establish a program to enable States to furnish services to support informal caregivers of individuals with functional disabilities.

Outlines State plan requirements for such caregiver support services, including requirements that: (1) States must support 50 percent of program administration costs; (2) services may not supplant Medicare or Medicaid (titles XVIII and XIX of the Social Security Act) or private insurance otherwise available to eligible recipients of services; and (3) persons with incomes exceeding 200 percent of the Federal poverty level must pay on a sliding fee scale established by the State, up to a maximum annual income of $75,000.

Caps total annual Federal expenditures at $2,400 per eligible recipient.

Authorizes appropriations to carry out the new title.
